He has no health bar
You don't need to treat Bullet like a traditional companion in a video game. He has no health bar and he cannot die. It works the same as a tool in your tool belt that you can use freely without worrying about losing it. He is an ideal companion who can take care of himself. So, whenever you are in danger, you should focus on yourself, not on him.
Listen to him and observe his body language
Bullet is a trusted companion who knows right from wrong. As such, he wants to keep you safe and will let you know as soon as he gets close to you. When Bullet starts barking or you notice his body language change to adopt a particular posture, you want to take that into account. Due to his keen sense, he will have a greater amount of information.
Use your commands
You want to use the available commands when you call it to your advantage. You should have five available. Before you can use any of them, you need to call. To call Bullet into your position, for mouse and keyboard, you want to press "C", while those using a controller have to press the LB button.
When Bullet has attention and he's in front of you, watch him. You then want to hold down the "C" or LB key to display the various options available to you. You are going to have:

To help bolster Ellis' sanity, you can call him Bullet and make him Pet Bullet, which makes things a little better. It won't fix the situation, but Bullet helps Ellis stay the course and see it through.

Bullet has a lot of shoulder weight being Ellis' mate as they both fight the witch. As such, he deserves good treatment. As you play the game you are going to find treats around the forest that you can give Bullet to be a good boy. When you find them, you can access them from your inventory to give them to Bullet.
